Please check the boxes below to confirm your understanding and agreement with group requirements and select your payment preference.

1. While this is a group specifically for mental health professionals, this is not a space for clinical consultation or supervision. While clinical topics may arise, the focus of the group is to connect with others outside of our clinical roles and responsibilities.

2. “Social Stitching” is a non-clinical and non-therapeutic setting and is not to be treated as a replacement for therapy or treatment of any kind. Even so, sensitive personal information may be shared during meeting times and is expected to be held in confidence.

3. Fees will be collected via stripe through Seeds of Change, PLLC’s simple practice account. You will be asked to save a credit card on file that will be charged according to the payment option you select. You will only be asked to complete a credit card authorization form and input your card information into this account.

4. This agreement along with your name, phone number, and email address are the only pieces of information outside of what is needed for processing payment that will be in your simple practice account. Your account will be clearly identified for administrative purposes as a social membership rather than a client account.

5. If you decide to use the “pay as you go” installment option, your financial responsibility is not dependent on your attendance and participation as it is not a “fee for service” charge. You will be charged $10 the morning of each stitching session.

"Pay as you go" Installment Plan ($30 deposit charged to reserve spot and cover materials and shipping, $10 charge each morning of the 6 meeting dates; total of $90)

You will get an email invitation to upload your credit card information through simple practice. Once your deposit or full enrollment fee has been charged, you will be added to the google calendar event and your cross-stitch kit will be mailed within 72 hours.
